Old game, don't know title

Ok, I'm looking for an old game that I used to play when I was in preschool, kindergarten, or maybe early elementary. (I'm 21 now). You had to reproduce a shape with holes in it, is the best I can describe it. For example, you started out with a square, and you could rotate it and punch holes in it to get to the final product. You gad to rotate and stamp or punch it in the correct order or it wouldn't come out correctly. It is not the incredible machines series. I believe it was a "sunburst" game, or something like that, and it was for (I think) and Apple II (Green screen, literal floppy disks.) If anyone could help me out, even if you just know the title, and nothing else, that would be great. Heck, if you even recognize the concept from what I've told you, that would be great. I'm beginning to think I imagined the game. lol

Ok, this is a very, very rough drawing of what I remember of the game. In my drawing, for example, you have to rotate it 45 degrees, punch a square hole in the middle, and rotate it 45 degrees again. Does anyone have any idea what the heck I'm talking about?? I really am beginning to think I made the game up. lol
